What is Transition House?

Transition House (TH) is a five bedroom house in Central Point built with grant funds from Housing and Urban Development, leased to The Arc of Jackson County through Housing Authority of Jackson County. The basic program involves four individuals with developmental disabilities living in the five bedroom house with a paid live-in assistant.

What is the purpose of Transition House?

The purpose of TH is to give individuals with a development disability an opportunity to live semi-independently while learning life skills needed to transition into independent living.

What assistance does the Transition House assistant provide? 

The assistant is available one-hour and 15 minutes in the morning and evening, Monday through Sunday with the exception of every other weekend. (The pay is covered by the $145 per individual per month.) During these hours the assistant assists with meal preparation, personal care, and skills training. The assistant is also responsible for organization of routine household tasks and modeling appropriate socialization in communal living. Additional assistance by the assistant is available and may be paid for using the individual's 20 Hour Personal Care Dollars through the State of Oregon. Gaps not covered by the 20 hours may be paid for by the family using their own resources.

What are the eligibility requirements? 

Individuals must have a developmental disability attributed to mental and/or physical impairment, mental retardation cerebral palsy, epilepsy or other neurological disorder originating prior to age 22, expecting to continue indefinitely. Individuals must also have an income no more than $13,600 (under or at 50% of median income level).

What criteria must the individual meet? 

All individuals must be able to remain home alone; identify and respond to emergencies and get help; respond to a stranger at the door and with discretion; able to provide own basic nutrition/hydration; and have no history in the past two years of intimidation, coercion, harassment, assault or major property destruction.

What is the cost? 

$435 per month ($290 for rent and utilities and $145 for the assistants pay) food and personal care items are not included. There is a $250 refundable security deposit.

How do I move in? 

1. Contact The Arc to let them know you are interested. (You will be given an "evaluation tool" to assess the individual's current level of independence, Transition House Policies and the phone number of the TH assistant, and requested to release records for The Arc to review.)
2. Contact the assistant in order to view the home and meet the assistant .
3. Meet with the participants and their families for final acceptance .
4. Contact the Housing Authority of Jackson County to determine income eligibility                                                            5. Once accepted, fill out The Arc's Rental Agreement,and pay deposit and rent.
