ELIGIBILITY: Medically uninsured residents and their families who have limited ability to pay for timely health care. People who have a job without employer sponsored insurance and make just a little too much money to qualify for the Oregon Health Plan. People who have language, cultural or other non-financial barriers to obtaining health care.
SERVICES:
- Primary health care is basic medical care encompassing curative and preventative medicine, health promotion and maintenance, education, nutrition and continuing care evaluation and management. Provided on an out-patient basis, primary care services as the critical entry point to the health care delivery system. Services include:
- Diagnosis & treatment of illness
- Screening & disease prevention
- Well child exams & Immunizations
- Women's health exams
- Physical examinations
- Management of chronic illness
- Health education
- Prenatal care (White City Clinic)
- Specialty clinics
FEES:
Sliding fee based on income. Accepts Medicaid, and OHP, fee is payable upon visit.
